难治性肺炎支原体肺炎患儿临床特点及危险因素分析

Clinical characteristics and risk factors of refractory Mycoplasma pneumoniae pneumonia in children

摘要:  目的 探讨难治性肺炎支原体肺炎(RMPP)患儿的临床特点及相关危险因素。方法 回顾分析2012年1 月至2016年10月收治的628例2~12岁肺炎支原体肺炎(MPP)患儿的临床表现、实验室指标及影像学特点,采用逐步 logistic回归分析RMPP发生的相关危险因素,绘制ROC曲线分析各独立危险因素在RMPP中的预测价值。结果 与486 例普通型支原体肺炎(GMPP)相比,142例RMPP患儿的年龄更大,热程、住院时间、阿奇霉素治疗天数更长,肺外并发症、 右肺病变、右上肺病变、胸腔积液、大叶性肺不张、肺部大片实变影及胸膜增厚的比例更高,中性粒细胞百分比、 C反应蛋 白(CRP)、乳酸脱氢酶(LDH)、肌酸激酶同工酶(CK-MB)、白介素(IL)-6、IL-10、干扰素-γ( IFN-γ)及总IgA水平也更 高,差异均有统计学意义(P<0.05)。 逐步logistic回归分析显示,CRP、LDH、IL-6、IL-10和IFN-γ是RMPP的独立危险 因素。ROC曲线分析显示,CRP、LDH、IL-6、IL-10及IFN-γ用于鉴别RMPP和GMPP患儿具有较好价值,其最佳阈值分 别为15.3 mg/L、402 IU/L、13.82 pg/mL、5.07 pg/mL和13.84 pg/mL。结论 临床症状、肺部影像学表现加重,血清CRP、 LDH、IL-6、IL-10及IFN-γ水平明显升高有助于早期识别RMPP。

Abstract:  Objective To investigate the clinical characteristics of children with refractory Mycoplasma pneumoniae pneumonia (RMPP),and to analyze the related risk factors. Methods   Clinical manifestations,laboratory indexes and image features of 628 children with Mycoplasma pneumoniae pneumonia (MPP) from January 2012 to October 2016 were retrospectively analyzed, and stepwise logistic regression analysis was performed for risk factors of RMPP. ROC curve was mapped to analyze the predictive value of independent risk factors in RMPP. Results Compared with 486 cases of general Mycoplasma pneumoniae pneumonia (GMPP), 142 cases of refractory MPP was older, and durations of fever and hospitalization, azithromycin treatment days were longer, pulmonary complications, lesions in the right lobe and right upper lobe, pleural effusion, lobar atelectasis, pulmonary consolidations, and pleural thickening ratio were higher, the percentage of neutrophil, CRP, LDH, CK-MB, IL-6, IL-10, IFN-γ and IgA levels were also higher (all P<0.05). Stepwise logistic regression analysis showed that CRP, LDH, IL-6, IL-10, IFN-γ were the independent risk factors of RMPP. ROC curve analysis showed that CRP, LDH, IL-6, IL10 and IFN-γ has a good value for identification of children with RMPP and GMPP, the best threshold value of 15.3 mg/L, 402 IU/L, 13.82 pg/mL, 5.07 pg/mL, and 13.84 pg/mL, respectively. Conclusions  The clinical symptoms and imaging findings of the lung are severer,and the levels of serum CRP, LDH, IL-6, IL-10, and IFN-γ are significantly increased, which is helpful for the early identification of RMPP.
