血清miR-200c-3p 表达水平对判断新生儿呼吸窘迫综合征严重程度和预后的价值

The value of serum miR-200c-3p expression in determining the severity and prognosis of neonatal respiratory distress syndrome

摘要: 目的 探讨血清miR-200c-3p表达水平对判断新生儿呼吸窘迫综合征(NRDS)严重程度和预后的价值。方 法 选取2016年6月—2019年6月收治的102例NRDS新生儿,应用新生儿急性生理学评分围生期补充II(SNAPPE-II)评 分进行评估。根据患儿生存情况分为生存组(n=73)和死亡组(n=29),按照X线结果和病情严重程度分为轻度组(Ⅰ、Ⅱ 级,67例)和重度组(Ⅲ、Ⅳ级,35例),采用实时荧光定量PCR检测各组血清miR-200c-3p表达水平。应用ROC曲线分 析miR-200c-3p表达水平及SNAPPE-II评分预测NRDS患儿死亡的价值。结果 死亡组血清miR-200c-3p表达水平及 SNAPPE-II评分均明显高于生存组,重度组miR-200c-3p表达水平、SNAPPE-II评分及病死率高于轻度组,差异均有统计 学意义(P<0.05)。ROC曲线分析显示,血清miR-200c-3p表达水平联合SNAPPE-II评分预测NRDS死亡的曲线下面积 (AUC)为0.93 (95%CI:0.87~0.98),明显高于单项miR-200c-3p(0.84,95%CI:0.78~0.90)及SNAPPE-II评分(0.80, 95%CI:0.75~0.86),差异有统计学意义(P<0.05),其灵敏度和特异度为94.0%和88.5%。NRDS患儿血清miR-200c3p表达水平与SNAPPE-II评分呈显著正相关(r=0.84, P<0.01)。 结论 血清miR-200c-3p表达水平升高与NRDS病情严 重程度及预后相关,miR-200c-3p联合SNAPPE-II评分对NRDS预后判断具有较高价值。

Abstract: Objective To explore the value of serum mir-200c-3p expression in determining the severity and prognosis of neonatal respiratory distress syndrome (NRDS). Methods A total of 102 NRDS newborns admitted to the Second Affiliated Hospital of Hainan Medical College from June 2016 to June 2019 were selected and the Score for Neonatal Acute Physiology with Perinatal Extension-II (SNAPPE II) was calculated. According to the survival of the children, they were divided into the survival group (n=73) and the death group (n=29). According to the X-ray results and the severity of the disease, the children were divided into the mild group (grade Ⅰ and Ⅱ, 67 cases) and the severe group (grade Ⅲ and Ⅳ, 35 cases). The expression level of miR-200c-3p was detected by real-time fluorescent quantitative PCR. ROC curve was used to analyze the miR-200c-3p levels and SNAPPE-II score in predicting death in children with NRDS. Results The serum miR-200c-3p expression level and SNAPPE-II score in the death group were significantly higher than those in the survival group, the expression level of miR-200c3p, SNAPPE-II score and mortality rate in the severe group were significantly higher than those in the mild group, and differences were statistically significant (P<0.05). ROC curve analysis showed that the area under the curve (AUC) of serum miR-200c-3p expression combined with SNAPPE-II was 0.93 (95%CI: 0.87~0.98) in predicting NRDS death, significantly higher than that of miR-200c-3p alone (0.84, 95%CI: 0.78~0.90) or SNAPPE-II alone (0.80, 95%CI: 0.75~0.86), and differences were statistically significant (P<0.05). Its sensitivity and specificity were 94.0% and 88.5% respectively. There was a significant positive correlation between the expression of miR-200c-3p and SNAPPE-II in NRDS (r= 0.84, P< 0.01). Conclusions The increased expression of miR-200c-3p in serum is related to the severity and prognosis of NRDS. The combination of miR-200c-3p and SNAPPE-II is of high value in the prognosis of NRDS.
